A COMPLETED, NOTARIZED EMERGENCY
CARE RELEASE FORM
This form must be notarized and signed by
both guardians before your child will be allowed to attend school.
A COMPLETED HEALTH FORM
This form includes general information,
up-to-date immunizations, and a physical exam.
Please be sure to complete the entire form.
A COMPLETED STATEMENT OF FINANCIAL
RESPONSIBILITY AND PARENTAL AGREEMENT FORM
A COMPLETED INCOME ELIGIBILITY FORM
*It is very important that all your forms
are complete. We understand that this
paperwork seems repetitive at times, but we appreciate you taking the time to
provide the school with the most up-to-date and accurate record possible for
your child.
FIRST TUITION PAYMENT
Be sure to either mail or hand deliver your
payment to office staff. Please do not
simply leave payment on the desk; you may deposit it in our locked box marked
for tuition. Tuition payments are always due on the first day of the
month. Please include your child’s name
on the envelope and/or check.
TOOTHBRUSH
Please label with your child’s name.
TWO COMPLETE CHANGES OF CLOTHES
Provide small shoebox or equivalent,
labeled with child’s name. Label each
item with child’s full name in permanent marker.
RESTING THINGS
North Room Children: Provide two small cot-sized
towels in pillowcase labeled with
child’s
name for nap time. No heavy blankets or
pillows.
East, West, and South Room Children:
Provide one lightweight towel or blanket in
pillow
case for rest time after lunch. Please
label with child’s full name.
All
resting things must be taken home every Friday to be washed. Sick children must
also
take their bedding home for washing.
